REGISTRATION/PATIENT FORMS
New patients may complete their paperwork through the patient portal or download and complete the New Patient Forms available in the Patient Information section. Please complete your forms before your initial appointment and bring them with you or fax them to our office at 740-387-3668. Please also bring a current photo ID, insurance card and an updated list of your medications.
INSURANCE
We participate with Medicare and many major insurance plans. Because participation can vary by specific plan and network, we recommend confirming coverage with your insurance carrier before your appointment. Please bring your current insurance card and photo ID to each visit. Deductibles, copayments and coinsurance are due at the time of service.
REFERRALS
We know that a referral is the highest compliment we can receive. We are committed to giving you the best possible care and take pride in our professionalism and the team effort of our expertly trained staff. New patients are always welcome. If your insurance company requires a referral, please make sure our office receives the referral information prior to your appointment.
AFTER-HOURS & EMERGENCY CARE
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Established patients with an urgent foot or ankle concern outside normal office hours may call our office at 740-383-5115 for information regarding after-hours assistance.
