REGISTRATION/PATIENT FORMS
For new patients, please fill out new patient forms located under the patient information section before your initial visit. This will allow us to provide you with appropriate care and keep from a longer office visit. Please bring these forms to your office visit or fax them to us before your appointment at (740) 387-3668.
INSURANCE
Drs. Brown are participating providers for Medicare and most insurance plans. Our billing department automatically processes your claims. To do so, we need your signature or authorization on file, the name and address of your insurer and the name and I.D. number of the insured person. Please bring your insurance card at the time of your initial visit and again if your plan should change. Payment for all deductibles, co-pays and co-insurance is due at time of service. Treatment of Minors requires in person consent from parent or legal guardian.
REFERRALS
We know that a referral is the highest compliment we can receive. We are committed to giving you the best possible care and take pride in our professionalism and the team effort of our expertly trained staff. New patients are always welcome. If your insurance company requires a referral, please make sure our office receives the referral information prior to your appointment.
EMERGENCY CARE
- If you have an urgent concern after normal business hours, please call our office for instructions on reaching the on-call provider (740) 383-5115.
- For a medical emergency, call 911 or go to the nearest emergency department. You may let the emergency department know that you are a patient of Dr. Brown’s office.
CANCELLATIONS
Please be kind enough to notify us 24 hours in advance so we can accommodate the schedules of other patients.
