Once your appointment has been confirmed, please complete your new-patient paperwork before your visit. You may:
- Complete the forms through the Patient Portal using the link below, or
- Download and complete the forms under the Patient Information tab.
- Please bring your completed forms to your appointment or fax them to our office in advance 740-387-3668.
Please bring the following to your appointment:
- Your photo ID
- Your current insurance card
- A list of your current medications
- Any completed forms, if they were not submitted electronically or faxed in advance
